Under-Chin Fullness in Hawaii: What an Under-Chin Injection Can Do

An under-chin injection is a cosmetic treatment for fullness under the chin. Small injections break down fat cells in that one area. At our Kailua, Oahu clinic, Doug Williams, MPAS, PA-C, can help you see if it fits.

You catch your side profile in a photo from a beach day. The area under your chin looks fuller than you remember. Maybe diet and exercise have not changed it much. You are not alone, and there are options worth knowing about.

What causes fullness under the chin?

Fullness under the chin often comes from a small pocket of fat. Medical providers sometimes call it submental fat. It is common, even in people who are fit and active.

A few things play a part:

  • Family traits, since some people simply store fat there

  • Changes in weight over time

  • Skin that loosens with age and sun exposure

Not every full chin is fat. Loose skin or a weak jaw angle can look similar. That is why a real exam matters before any treatment.

How does an under-chin injection work?

The medicine copies a substance your body makes to digest fat. Placed under the chin, it breaks down the walls of fat cells. Over the following weeks, your body clears them away.

Here is what a visit usually looks like:

  • Your provider marks the area under your chin

  • A numbing step can help with comfort

  • Several tiny injections go into the marked area

  • The visit is often done within an hour

Most people need more than one session to reach their goal. Sessions are spaced weeks apart so the area can settle. Your provider will talk with you about a plan that fits your face.

Who is a good fit for an under-chin injection?

This treatment tends to fit adults with a moderate pocket of fat under the chin. It works best when the skin still has decent bounce.

Quick facts:

  • Treatment area: the fat under the chin only

  • Visit time: usually under an hour

  • Sessions: most people need more than one

  • Downtime: swelling for a few days is common

It may not be the right fit if most of the fullness comes from loose skin. It is also not for people who are pregnant or nursing. Some health conditions in the neck area also need a closer look first. Be open about your health history at your visit.

What should you expect after treatment?

Swelling under the chin is the most common side effect. Many people also notice tenderness, bruising, or a numb feeling for a while. These tend to fade over days to a couple of weeks.

Plan around it. If you have a wedding, a family party, or photos coming up, leave plenty of time. Most patients see changes slowly, over weeks, not overnight.

Call the clinic right away if you notice:

  • Trouble swallowing

  • A lopsided smile or weakness in your lower lip

  • Swelling that keeps getting worse after the first few days

For trouble breathing or severe swelling, get emergency care first.

Why does timing matter in Hawaii?

In Hawaii, your neck and jaw get sun all year. Swollen or bruised skin can be more sensitive to UV. That makes sun care part of your plan, not an afterthought.

A few Hawaii tips:

  • Wear a wide-brim hat on the beach or at the ballpark

  • Use a mineral sunscreen on your neck every day

  • Skip long ocean swims until the swelling goes down

  • Keep cool on humid days, since heat can make swelling feel worse

Fall is a good window for many people on Oahu. Summer travel is done, and holiday parties are still weeks away. That gives your skin time to settle before the busy season.

What are other options for a fuller chin?

An under-chin injection is one tool, not the only one. Hawaiian Islands Dermatology also offers CoolSculpt, a cooling treatment that targets fat without needles. Some people are a better fit for one or the other.

If loose skin is the bigger issue, ask about skin-tightening options instead.

Our pick: Luminous Repair Cream. It supports the skin barrier and skin quality on your face and neck. It is part of the Soleivar line, made for skin that has changed after 30.

What not to do

  • Do not buy injectables online or from anyone who is not licensed

  • Do not book treatment the week before a big event

  • Do not skip sunscreen on your neck while you heal

  • Do not press or massage the area unless your provider says to

When should you see a provider?

Book a visit if the fullness under your chin bothers you and you want a plan. A consult is also smart if you are not sure whether it is fat or loose skin.

See a provider sooner for a new neck lump, or one that grows or hurts. Those need a medical exam, not a cosmetic one. Our team sees medical and cosmetic patients, so we can check both.

Common questions

Does an under-chin injection hurt?

Most people feel a stinging or burning feeling during and shortly after the injections. A numbing step and cold packs can help. The feeling usually fades within the hour.

How many sessions will I need?

It depends on how much fat is there and your goals. Most people need more than one session, spaced weeks apart. Your provider will give you a plan at your consult.

Is the fat gone for good?

The treated fat cells are broken down and cleared by your body. Big weight changes later can still affect how your chin and neck look. Your provider will talk about what to expect for your face.

Can I go back to work the next day?

Many people do, as long as they are fine with some swelling. If you have a camera-facing job, you may want a few days of buffer. Plan around key photos and events.

Is this covered by insurance?

No. This is a cosmetic treatment, so insurance does not cover it. Cherry financing is available for cosmetic treatments.
